Ten Considerations for Successful Practice

Launching a private practice is both an exciting and challenging endeavor – requiring balance of medical expertise, business strategy, and patient-centered care!

Here are 10 considerations for building and maintaining a successful private practice:

  1. Clear Practice Model: Decide: general headache vs. migraine specialty vs. comprehensive headache center. Identify differentiators (e.g., infusion therapy, pediatric migraine, procedures). Choose: insurance-based, concierge, or hybrid.
  2. Credentialing and Insurance Contracts: If accepting insurance, decide which you’ll accept and plan ahead as credentialing can take months. You will also need to establish referral basis. Assess patient demographics, competition, and referral sources in your area.
  3. Financial Planning & Revenue Streams: Budget for startup costs (building rental vs. owning, equipment/supplies, EHR, staff salaries).Explore diverse revenue streams: office visits, in-office procedures (Botox, nerve blocks, infusions), research trials, telemedicine, consulting or speaking engagements. Stay on top of coding and billing compliance to avoid revenue loss.
  4. Legal Considerations: Liability insurance of every kind (malpractice, general liability, cybersecurity, payroll, personal/disability). Establish clear staff contracts and do background checks.
  5. Efficient Clinic Operations: Invest in a reliable EHR that supports neurology-specific needs (Botox and nerve block procedures, etc.).Optimize scheduling to balance new vs. follow-up visits vs. procedures. Use medical assistants, scribes, or advanced practice providers strategically.
  6. Staffing & Training: Hire for both competence and alignment with your mission. Provide training in patient communication and neurological procedures. Build a team culture that emphasizes empathy and efficiency.
  7. Patient Experience & Retention: Make scheduling, check-in, and billing patient-friendly. Keep wait times reasonable and communication clear. Consider patient portals and telehealth for convenience. Reputation (word-of-mouth + online reviews) is critical in neurology.
  8. Compliance & Risk Management: Stay up to date with HIPAA, OSHA, and CMS regulations. Implement a system for medication prior authorizations and appeals.
  9. Sustainability & Growth: Monitor financial performance and patient demand to know when to add providers or expand services (infusion suite, sleep medicine, pain psychology). Continuously track outcomes to demonstrate value to patients and referring providers.
  10. Work-Life Balance & Sustainability: Set realistic patient volume goals to prevent burnout. Build efficient systems so you can spend more time with patients (and less on admin). Consider partnerships or part-time associates as your practice grows.
